WHO QUALIFIES

Anyone who receives an email invitation from USClaims to participate in the survey and participates in the survey by no later than the last day of the month the email is sent will be automatically entered for a chance to win the award. Only those individuals who signed a contract with USClaims and to whom the email invitation to participate in the survey is addressed qualify to enter.
